Employment Timeline of 3 Years is required

• Minimum 9 month driving experience
• Moving Violations: No more than 3 in the past 3 years in a CMV
• No more than 2 in the last 12 months in a CMV DUI/DWI”s: 1 allowed, but must be outside of 7 years
• Automatic DQ”s: Rollover past 4 years
• Lane change accident past 4 years
• Side swipe past 4 years
• Rear end past 4 years
• Improper backing with accident past 3 years
